Position Summary: As a Program Manager you would be responsible for overseeing consumer services through leadership, staff supervision, case management, consumer advocacy, health and safety, and financial management.
Education: A high school diploma or GED is required. An associates or bachelor's degree in rehabilitation, psychology, social work or other related major is highly preferred.
Work Experience: Must have a minimum of 2 years' experience working with persons with developmental disabilities. Preference will be given for supervisory and/or case management experience.
General Requirements: Must have a valid driver's license, acceptable driving record, meet agency's driver insurability requirements, reliable vehicle, and up-to-date automobile insurance.
Working Conditions: This position requires significant bending, lifting and stooping. Must be able to lift/transfer individuals.
Contributing to the leadership of the organization through participation in the decision-making process,
Reviewing and evaluating the effectiveness of existing programs,
Supervising assigned personnel and assisting with staff training,
Ensuring the maintenance of a clean, safe and healthy environment for consumers,
Encouraging greater independence and self-reliance with consumers,
Assisting consumers in exercising choices and developing self-advocacy skills,
Developing and overseeing individualized consumer plans,
Ensuring that regular activities that are scheduled contribute to a meaningful day,
Overseeing consumer belongings and management of finances,
Ensuring medications are ordered in a timely manner, administered properly and documented accurately,
Providing personal care and transportation as needed,
Maintaining communication with consumers and their family members, support staff and other service providers,
Coordinating with health care professionals to ensure consumers' medical needs are met,
Providing on-call support and responding to emergency situations or providing crisis intervention as needed,
Recording, reporting, and auditing consumer documentation and financial information,
Responsible for meeting CARF, FSSA, BQIS, Medicaid, and Board of Health standards and other regulations and procedures.
